ADHD/ADD assessment including QBCheck, and if diagnosed the first prescription:

If you have received an ADHD/ADD from another clinician and wish to request medication or a prescription through Dr Mirkhani, the existing assessment and diagnostic report will need to be reviewed before any prescribing decision can be made.

Dr Mirkhani will need to establish that the assessment has been conducted to an appropriate clinical standard and that the diagnostic process and recommendations are consistent with current NICE guidance and accepted clinical practice. The report will therefore be reviewed in detail, including the assessment methodology, supporting evidence, diagnostic formulation, relevant medical and psychiatric history, and any recommendations regarding medication.

Please note that reviewing an external diagnostic report for the purpose of prescribing is not a simple prescription or administrative review. Dr Mirkhani remains professionally responsible for any medication he prescribes and must therefore be satisfied that there is sufficient clinical evidence to support the diagnosis and that prescribing is safe and appropriate.

For this reason, clients requesting prescribing following a diagnosis made by another clinician will be required to have a full initial assessment with Dr Mirkhani, charged at the standard initial assessment fee of £750. This applies even where a previous diagnosis has already been made.

Following the assessment, Dr Mirkhani will discuss whether medication is clinically appropriate and, where appropriate, agree a treatment and monitoring plan. A previous diagnosis does not automatically guarantee that medication will be prescribed. If the available information is insufficient, inconsistent with current clinical guidance, or further assessment is required, additional information or assessment may be recommended before prescribing can be considered.
